2015-2016 School Year Indiana State Department of Health School Immunization Requirements


Developmental Kindergarten – 4 DTaP; 3 Polio; 1 MMR; 3 Hepatitis B; 1 Varicella

Kindergarten – 5 DTaP; 4 Polio; 2 MMR; 3 Hepatitis B; 2 Varicella; 2 Hepatitis A

Grades 1 to 5 – 5 DTaP; 4 Polio; 2 MMR; 3 Hepatitis B; 2 Varicella

Grades 6 to 11 – 5 DTaP; 4 Polio; 2 MMR; 3 Hepatitis B; 2 Varicella, 1 Tdap (Tetanus & Pertussis); 1 MCV4 (Meningococcal conjugate)

Grade 12 - 5 DTaP; 4 Polio; 2 MMR; 3 Hepatitis B; 2 Varicella; 1 Tdap (Tetanus & Pertussis); 2 MCV4 (Meningococcal conjugate)


Recommended, but not required HPV series for both girls and boys.

**Children entering Developmental Kindergarten and Kindergarten MUST have their immunizations COMPLETED BEFORE school starts.

**Children entering 6th and 12th grade MUST have all immunizations COMPLETED BEFORE school starts.


Reminder: Indiana Health Departments no longer provide vaccinations to children with insurance coverage for vaccines. Children who do qualify for vaccines at the health department are those who have Medicaid, are Alaskan or American Indian, or are those with limited or no insurance coverage. Please call the school nurse with any questions concerning health requirements.
